For eligible DaVita teammates and dependents, the following are available to you at no cost as part of Ilant's program: regular visits with your clinical team and peer navigator, a connected device (varies by program), and access to Ilant's digital app and personalized content. Any labs/blood work ordered, medications prescribed, or surgical procedures referred to are not included in Ilant's program but are subject to your health plan coverage, deductibles and co-pays.

With Ilant, you will now have access to specialized clinicians. Physicians are board-certified in obesity medicine; lifestyle and behavioral support is provided by registered dietitians and licensed behavioral health clinicians that specialize in weight management, diabetes and heart health.

Starting July 15, 2026, Ilant Health services will be available to eligible DaVita teammates and dependents. Anyone seeking a new prescription for weight loss medications (such as Wegovy, Zepbound, Foundayo, phentermine, Qsymia or Contrave) must work with an Ilant physician. If you're already using a weight loss medication, you'll have until October 31, 2026 to continue filling your current prescription. Starting November 1, 2026, all prescriptions for weight loss medications must be prescribed and managed by Ilant providers. This means that after October 31, any prescription written by a non-Ilant provider will not be covered by Anthem.

Note: This requirement applies only to weight loss medications. GLP-1 medications prescribed for diabetes are not affected.

Ilant is a specialty care provider that will work closely with your existing care team. During onboarding, you provide your existing care team information, and Ilant will send a letter to your provider introducing our program. We share your notes and treatment plan via electronic medical records, and can do provider-to-provider consults where greater coordination is appropriate.

While you can absolutely continue working with your current providers, Ilant providers will need to be additionally included on your care team if you are looking for weight loss medication coverage. For those who wish to exclusively continue to work with their current providers and not with Ilant, they can pursue weight loss medication outside of their covered Anthem benefit (i.e. self pay).

In order to ensure evidence-based, best-in-class care, Ilant Health requires blood work to participate in its program. Ilant will order these labs for you during onboarding, but can also accept previously done labs within the last 6 months. The required tests include: CBC, CMP, Lipids, A1c, Iron, TSH, and Vitamin D.

Ilant Health offers eligible DaVita teammates and dependents four programs:

1. For those with a BMI under 27, you may be eligible for our Weight Management & Prevention program that supports members in developing healthy nutrition and behavioral habits.
2. For those with a BMI of 27 or above, you may be eligible for our Medical Weight Loss program, which may combine lifestyle changes with a medical treatment such as medication or bariatric surgery.
3. For those with long-term diabetes, we offer a Diabetes Management program.
4. For those with long-term heart conditions, such as hypertension or coronary artery disease, we offer a Cardiovascular Management program.

The ultimate decision on the right program and treatment for you will be made in a discussion between you and our clinicians, taking into account your medical needs and personal preferences.

Our Weight Management & Prevention and Medical Weight Loss programs include a connected smart scale. The Diabetes Management program includes a connected glucometer. The Cardiovascular Management program includes a connected blood pressure monitor. Additional devices may be included based on clinical need.

Ilant Health is an evidence-based, medical obesity solution that supports you in every step of your journey. We prescribe FDA-approved medications, as appropriate, along with personalized support from board-certified obesity medicine physicians, licensed behavioral health clinicians, registered dietitians, and peer navigators. You'll have regular video visits with your care team, and support through messaging and access to educational tools through the Ilant app.

Ilant doctors can prescribe FDA-approved branded and generic medications for prediabetes, obesity, high blood pressure, and high cholesterol. Based on your medical history, lab tests, and insurance coverage, our Care Team will determine if medication is appropriate and which medication is most effective for you. For weight loss medications, such as Zepbound and Wegovy, among others, Ilant is the exclusive prescriber for DaVita teammates and dependents, meaning that for these medications to be covered, they must be prescribed only by Ilant providers. If a non-Ilant provider prescribes weight loss medication, the medication will not be covered by your Anthem plan.

Ilant's program is available as a covered benefit to those DaVita teammates and dependents enrolled in a DaVita-sponsored Anthem health plan and are 18 years old or above.

Weight loss can help reduce or even reverse the following conditions: Diabetes, Sleep Apnea, High Blood Pressure, High Cholesterol, Heart Disease, and Joint Pain.

Ilant members, on average, see weight loss of 15% after 12 months on program. They also see significant improvement in other health indicators such as A1c, LDL, Triglycerides, and systolic and diastolic blood pressure. Many Ilant members have been able to come off other medications as these conditions improve. In addition to their health benefits, Ilant members report increased energy and the ability to engage in new activities.

DaVita covers weight loss medications for members enrolled in their health plans. Starting August 3, 2026, Ilant Health will be the exclusive prescriber for weight loss medications. Anthem members with a current prior authorization for weight loss medications will have until October 31, 2026 to transition their care to Ilant Health. Please note, Mounjaro and Ozempic for diabetes can be prescribed by Ilant and non-Ilant providers.

As of July 15, 2026, DaVita is replacing Omada and Wondr with Ilant Health's comprehensive and specialized cardiometabolic program. This provides one place for teammates' cardiometabolic needs. In addition to a connected device, you will now have access to specialized doctors, dietitians, and behavioral health clinicians who focus specifically on weight management, diabetes and heart health — at no cost to you.

No. Ilant Health does not share your personal health information — including your diagnosis, treatment plan, or medications — with your employer. Your health information is used only by your Ilant Health care team to provide your care, in accordance with HIPAA and Ilant's Notice of HIPAA Privacy Practices. DaVita, as your employer and plan sponsor, does not have access to your individual health records or clinical details.
