How It Works
1. Complete enrollment and in-take forms. Sharing your medical and family history, your daily challenges and goals, will enable Ilant to create a plan personalized for you.
2. Meet with your peer navigator. Our peer navigators have all lived obesity experience and can provide you ongoing support throughout your journey.
3. Meet with your clinical team. This includes board-certified obesity medicine physicians, and licensed specialists in nutrition and mental health.
4. Regular engagement through online App check-ins and care team visits. In between visits, you'll have access to peer navigators, a nurse line, and in-app resources.

FAQs
For eligible Amtrak employees and dependents, the following are available to you at no cost as part of Ilant's program: regular visits with your clinical team and peer navigator, a connected scale, and access to Ilant's App and educational content. Any labs ordered, medications prescribed, or surgical procedures referred are not included in Ilant's program but are subject to your health plan coverage, deductibles and co-pays.
Ilant Health is an evidence-based, medical obesity solution that supports you in every step of your journey. We prescribe FDA-approved medications, as appropriate, along with personalized support from board-certified obesity medicine physicians, licensed mental health clinicians, registered dietitians, and peer navigators. You’ll have regular video visits with your care team, and support through messaging and access to educational tools through the Ilant Member App.
If you and your Ilant doctor decide to pursue anti-obesity medication as a treatment, you may be prescribed medications such as Wegovy, phentermine and topiramate, metformin, and bupropion and naltrexone. Any prescription will still be filled by Caremark through their preferred pharmacy.
Ilant members are not guaranteed a prescription to medication unless it is determined a necessary part of their treatment plan.
Amtrak covers Wegovy for members enrolled in their health plans. Starting February 1, 2026, anti-obesity GLP-1 medication will only be covered by your Amtrak health plan if prescribed by an Ilant doctor.
Ilant’s program is a benefit available to all Amtrak employees and covered dependents age 18 and older who are enrolled in an Amtrak-sponsored Aetna health plan.
Starting January 1, 2026, Ilant Health services will be available to eligible Amtrak employees and dependents. Beginning February 1, 2026, anyone seeking a new prescription for anti-obesity GLP-1 medications (such as Wegovy) must work with an Ilant physician. If you're already using a GLP-1 for obesity, you’ll have until March 31, 2026 to continue filling your current prescription. Starting April 1, 2026, all prescriptions for anti-obesity GLP-1s must be managed through Ilant.
Note: This requirement applies only to GLP-1s prescribed for obesity. GLP-1 medications prescribed for diabetes are not affected
Ilant members, on average, see weight loss of 15% after 12 months on program. They also see a 50% improvement in biomarkers such as A1c, LDL, Triglycerides, and a reduction in systolic and diastolic blood pressure by 15 and 10, respectively.
Weight loss can help reduce or even reverse many conditions, including diabetes, sleep apnea, high blood pressure, high cholesterol, heart disease, and joint pain. Ilant Health offers three programs:
- For those with a BMI under 27, you may be eligible for our Prevention Program that supports members in developing healthy nutrition and behavioral habits.
- For those with a BMI of 27 or above, you may be eligible for our Obesity Management program, which may combine lifestyle changes with a medical treatment such as medication or bariatric surgery.
- For those with long-term diabetes, we also offer a Diabetes Management program.
